# Your Zepbound tracker, from one shot day to the next

> A Zepbound tracker for your weekly shot: log the dose and injection site, note symptoms in seconds, follow your weight and bring a clear record to appointments.

Canonical page: https://handlehealth.co/zepbound-tracker · Category: Trackers · Last checked against sources: 2026-09-24

Record each Zepbound shot and where it went, from a pen, a vial or a KwikPen. Answer a short daily check-in and keep your weight, food and fluids beside it. The record is free, and Handle never tells you what dose to take.

## In short

- A Zepbound tracker records each weekly shot (the amount, the day and the injection site) alongside how you feel and your weight trend, so appointments start from facts, not memory.
- Zepbound is tirzepatide from Eli Lilly, a once-weekly injection approved in the US for long-term weight management in adults with obesity, or overweight with a weight-related condition, and for moderate to severe obstructive sleep apnea in adults with obesity.[1]
- It comes in six strengths from 2.5 mg to 15 mg, as single-dose pens and vials and, since January 2026, as four-dose KwikPens and multi-dose vials.[1]
- In the 72-week SURMOUNT-1 trial of adults without diabetes, average weight loss was 15.0% to 20.9% depending on the dose, against 3.1% on placebo, counting everyone who started (the treatment-regimen estimand).[1][4]
- Handle records all of this for free, with no streaks or scores, and never tells you to change, skip or catch up a dose.

## What to record in a Zepbound tracker

Keep it small enough to keep up. These details are the hardest to remember later and the most useful at appointments.

- **Every shot, as it happened.** The amount, the day and the time. A late or missed week belongs in the record too; it is information, not a mark against you.
- **Where it went.** The label lists the abdomen and thigh, or the back of the upper arm if someone else injects, and asks you to rotate sites with each dose.[1]
- **Which pen or vial.** Note the date if you change form, because single-dose and four-dose forms have different storage rules.[1]
- **How you feel.** Nausea, diarrhea, constipation, tiredness. In the trials, most people who stopped Zepbound because of side effects did so in the first few months, mainly because of stomach symptoms.[1]
- **Food, fluids and protein.** Zepbound reduces appetite and food intake.[2] Fluids matter when your stomach is unsettled: the label warns that fluid loss can injure the kidneys.[1] See our [GLP-1 food tracker guide](https://handlehealth.co/glp-1-food-tracker) and our guide to [what to eat on Zepbound and Mounjaro](https://handlehealth.co/what-to-eat-on-mounjaro).
- **Dates that matter.** Starting, each dose change, any new medicine. On the contraceptive pill? The label recommends a non-oral method, or an added barrier method, for 4 weeks after starting and after each dose increase.[1]

### Shot day, logged in a few taps (in Handle)

Set up Zepbound once, with the amount your prescriber set and your shot day. After that, logging is a quick confirmation of the time you injected and the site: abdomen, thigh or upper arm, left or right.

- Amount, time and injection site in one entry
- Each dose in your record lists its site, so rotating is easy to follow
- Forgot to log it? Add it later, up to 30 days back
- Shot-day reminders in the iPhone app

## Pens, vials and KwikPens: one record for all of them

Zepbound was approved in November 2023.[2] Lilly began selling single-dose vials for self-pay through LillyDirect in August 2024, and in June 2025 announced vials in every strength.[5][6] Two four-dose forms, a multi-dose vial and the KwikPen, joined the label in January 2026.[1][7]

The label says an opened KwikPen or multi-dose vial is thrown away after a total of 30 days at room temperature, 30 days after first use, or four weekly doses, even with medicine left, and that a KwikPen is never shared, even with a new needle.[1] Handle's private note for the day is a handy place to write "new KwikPen today".

### How you feel, in a few taps a day (in Handle)

Handle asks about nausea every day, plus the symptoms you switch on, such as diarrhea or constipation. Add up to six more, like belching and fatigue, both in Zepbound's side-effect table.[1] More in our [side-effect tracker guide](https://handlehealth.co/glp-1-side-effect-tracker).

Some answers, such as severe nausea, come with a clear next step, like contacting your care team. Handle never diagnoses and is not a monitoring or emergency service; if you feel very unwell, contact your clinician or emergency services yourself.

- The same short card every day, whatever you answer
- A private one-line note for the day that nothing reads
- Missed a day? Fill it in later, up to 30 days back

## Side effects to watch on Zepbound

From Zepbound's label: SURMOUNT-1 and SURMOUNT-2, in which 2,519 adults took Zepbound for up to 72 weeks. Each figure is the share of people who reported it.

| Side effect | Placebo | 5 mg | 10 mg | 15 mg |
|---|---|---|---|---|
| Nausea | 8% | 25% | 29% | 28% |
| Diarrhea | 8% | 19% | 21% | 23% |
| Constipation | 5% | 17% | 14% | 11% |
| Vomiting | 2% | 8% | 11% | 13% |
| Stomach pain | 5% | 9% | 9% | 10% |
| Indigestion | 4% | 9% | 9% | 10% |
| Injection-site reactions | 2% | 6% | 8% | 8% |
| Fatigue | 3% | 5% | 6% | 7% |
| Belching | 1% | 4% | 5% | 5% |

*Source: Zepbound prescribing information, Table 1.[1] Side effects led 4.8% to 6.7% of people on Zepbound to stop, against 3.4% on placebo.[1] More in [Zepbound side effects](https://handlehealth.co/zepbound-side-effects).*

> **Get medical help straight away if you have**
> - Severe pain in your stomach area that won't go away, with or without vomiting. The label warns about acute pancreatitis.[1]
> - Signs of a serious allergic reaction, such as swelling of your face, lips, tongue or throat, or difficulty breathing or swallowing.[1]
> - Nausea, vomiting or diarrhea that does not go away. The label warns that fluid loss can injure the kidneys.[1]

### Weeks of Zepbound, side by side (in Handle)

Journey draws your weight as weekly averages with your Zepbound dose as a step line beneath it, beside how you felt, what you ate and drank, and your training. It never claims one explains another.

- No streaks, scores or reminders to weigh
- Resistance and cardio sessions counted separately
- Protein and fluid totals, with a target only if your clinician set one

## What the SURMOUNT trials found

All four trials enrolled adults with obesity or overweight. Averages describe large groups, not you, and trials of different people can't be compared directly.

| Trial | Who took part | Length | Average result, Zepbound vs placebo |
|---|---|---|---|
| SURMOUNT-1 | 2,539 adults without diabetes | 72 weeks | Weight −15.0% (5 mg), −19.5% (10 mg), −20.9% (15 mg) vs −3.1%[1][4] |
| SURMOUNT-2 | 938 adults with type 2 diabetes | 72 weeks | Weight −12.8% (10 mg), −14.7% (15 mg) vs −3.2%[1] |
| SURMOUNT-OSA | Adults with moderate to severe sleep apnea, in two trials | 52 weeks | Breathing interruptions in sleep (AHI) fell by 25.3 and 29.3 an hour, vs 5.3 and 5.5[1][8] |
| SURMOUNT-4 | 670 adults after 36 weeks on tirzepatide | 52 more weeks | Continuing: a further −5.5%; switched to placebo: +14.0%[1] |

*SURMOUNT-1 and SURMOUNT-2 figures count everyone randomized, whether or not they kept taking it (the treatment-regimen estimand).[1][4] The label's SURMOUNT-4 figures also count everyone randomized at week 36.[1] If your weight changes direction, that is a signal to discuss with your prescriber, not a failure. For the head-to-head trial against semaglutide, see [Zepbound vs Wegovy](https://handlehealth.co/zepbound-vs-wegovy).*

## A week with your Zepbound tracker

There is no right way to do this; here is one ordinary week.

- **Once — Pick your shot day.** Agree a day with your prescriber that fits your week. The label allows any time of day, with or without meals.[1] Reminders in the iPhone app can mark the day.
- **Shot day — Log it while it's fresh.** Record the amount and the site as soon as you've injected. Next week, your record shows which site you used.
- **Every day — Check in, and weigh if you like.** Ordinary days show what normal looks like for you, and weekly averages mean one heavy morning doesn't dominate your trend.
- **If a week goes differently — Record what happened.** Zepbound's prescribing information and Medication Guide have their own rule for a missed dose, so ask your prescriber or pharmacist. Handle never tells you to catch up.
- **Before an appointment — Bring the weeks with you.** Open Journey together. The consultation report, a Premium feature, is built for this moment: a summary your clinician can read without an account.

### A summary built for your appointment (Handle Premium)

The consultation report is a summary for your next visit: your Zepbound record including dose changes, your weight history, symptom summaries, nutrition and activity, and an optional note about what you want to discuss. It helps your clinician see the pattern quickly. More in our guide to [preparing for a GLP-1 doctor appointment](https://handlehealth.co/glp-1-doctor-appointment).

- Shared as a secure link that can be revoked at any time
- Your private daily notes are never included
- Your clinician needs no account

## Choosing a Zepbound tracker: log, sheet or app

Searching for a Zepbound tracking log, journal or spreadsheet? Any of them works if you keep it up; they differ in how much work it takes to turn entries into something readable. Our [GLP-1 tracker guide](https://handlehealth.co/glp-1-tracker) compares apps.

| | Paper log or journal | Spreadsheet | Handle |
|---|---|---|---|
| Shot, time and site | Written by hand | Typed each week | One entry, with your plan filled in |
| Symptoms | Whatever you jot down | A column you fill in | A short daily check-in |
| Weight trend | Numbers on a page | A chart, if you build one | Weekly averages, charted |
| Food, fluids and protein | If you write it down | Another sheet | Daily totals |
| Reminders | None | Set up elsewhere | Dose and check-in reminders in the iPhone app |
| For your prescriber | The notebook itself | Print or share the file | Consultation report link (Premium) |
| Cost | A notebook | Free | Free; some features are Premium |
| Platforms | Wherever you carry it | Any device with the file | iPhone |

*A paper log has real strengths: it needs no phone and no account, and it stays entirely private. If you'd like one, our [free printable GLP-1 tracker](https://handlehealth.co/printable-glp-1-tracker) is ready to use. See [what Handle's free plan includes](https://handlehealth.co/free-glp-1-tracker).*

## Questions people ask

### Is there a Zepbound tracker app?

Yes. Handle works with Zepbound: it records each weekly shot and injection site, a short daily check-in, your weight, food and fluids, and a consultation report for your prescriber is among its Premium features. The iPhone app is free to download from the App Store. For other options, see our [guide to GLP-1 trackers](https://handlehealth.co/glp-1-tracker).

### Does Lilly have its own Zepbound app?

Yes. Eli Lilly's free Lilly Health app says it supports people on Zepbound, Mounjaro or Foundayo. Its App Store listing, seen on September 23, 2026, describes a logbook, dosing reminders and charts to share with your healthcare provider, for US residents aged 18 and over.[11] Handle is a separate app that records any GLP-1 medicine.

### Is Handle's Zepbound tracker free?

Yes. Logging shots and injection sites, the daily check-in, weight, food, drinks and activity, and the Journey charts are all free, with no advertising. The Between doses estimate, the consultation report with share links, food photo scanning and iOS widgets are Premium features. You never need them to keep a complete record.

### Can I use a spreadsheet as a Zepbound tracker?

Yes, if you keep it up. Give it columns for the date and time, the amount, the pen or vial, the injection site (left or right), symptoms and how strong they were, weight, and anything new such as a dose change. What a spreadsheet won't do on its own is remind you, chart weekly weight averages, or produce a summary your prescriber can read quickly.

### Where should I inject Zepbound?

Zepbound's label lists the abdomen or thigh, or the back of the upper arm if another person gives the injection, and says to rotate injection sites with each dose.[1] Handle records the site, left or right, with every shot, so your record shows where you went last time. If one area keeps reacting, mention it to your care team.

### How long does Zepbound stay in your system?

Tirzepatide's half-life is about five to six days in people with overweight or obesity, and a steady level builds after about four weeks of weekly doses.[1] Handle's Between doses view, a Premium feature, estimates your level from the doses you recorded. It is a model, not a blood test, and it only draws doses you have already taken. Our guide to the [Zepbound and Mounjaro half-life](https://handlehealth.co/mounjaro-half-life) explains what the labels say.

### Is Zepbound the same as Mounjaro?

They contain the same medicine, tirzepatide, made by Eli Lilly. In the US, Zepbound is approved for weight management and sleep apnea, and Mounjaro for type 2 diabetes.[1][10] In the UK and EU there is no Zepbound brand, and Mounjaro carries the weight-management license.[9][12] Compare them in [Mounjaro vs Zepbound](https://handlehealth.co/mounjaro-vs-zepbound).

## About Handle

Handle is a GLP-1 treatment companion. It keeps your dose record, a short daily check-in, weight, food, drinks and activity in one calm place, and turns them into a summary you can choose to share with your clinician.

Handle supports recording and conversations with your care team. It does not diagnose, and it never tells you to start, change, skip or stop a dose. This page is general information, not medical advice. Always follow your prescriber and your medicine's instructions for use.
