- Tracking Ozempic means a short, honest record of which pen strength you used and when, where you injected, how your stomach coped, and how your weight moved over weeks rather than days.
- Ozempic (semaglutide) is a once-weekly injection from Novo Nordisk, in pens that deliver 0.25 or 0.5 mg, 1 mg or 2 mg; in the US it is approved for adults with type 2 diabetes, including to lower the risk of major heart events or worsening kidney disease in some of them.1
- Semaglutide's half-life is about a week, and steady levels build over 4 to 5 weeks of weekly doses.1
- Since January 2026, Ozempic has also been the US name for once-daily semaglutide tablets in 1.5, 4 and 9 mg strengths, which come with their own rules for taking them.2,3

What to record in an Ozempic tracker
A handful of details answers most of what comes up at a review.
- The dose and the day. Ozempic is taken on the same day each week, at any time of day, with or without meals, so the day anchors your record.1
- Which pen or syringe. Pens deliver 0.25 or 0.5 mg, 1 mg or 2 mg, and single-dose syringes exist too. Noting the device makes a change of strength easy to spot.1
- The injection site. Abdomen, thigh or upper arm, with a different spot each week if you use the same area.1
- How your stomach feels. In the label's trials, most reports of nausea, vomiting and diarrhea came during dose escalation, so the weeks after a step up deserve a note.1
- Your other medicines. With insulin or a sulfonylurea, low blood sugar is more likely and your prescriber may adjust those medicines; Ozempic also slows stomach emptying, which may affect how other medicines taken by mouth are absorbed.1

Between doses: an estimate of your week
Semaglutide lingers: the label gives a half-life of about a week, with steady levels after 4 to 5 weeks of weekly doses.1 Between doses turns the Ozempic injections you've logged into an estimated curve of that rise and fall, worked out from semaglutide's published absorption and half-life.
Treat it as an illustration: it is a model, not a blood test, it draws only injections that have already happened, and it gives no dose advice.

Ozempic injection sites and rotation
The label lists three areas: the abdomen, the thigh and the upper arm. If you inject in the same area two weeks running, it asks you to use a different spot within it.1 Handle's site picker covers all three, left and right.
Injection-site reactions such as discomfort or redness were reported by 0.2% of people on Ozempic in the label's placebo-controlled trials.1 Still, a log makes rotating easy to follow and gives your care team something concrete if one spot keeps reacting.
If you also use insulin, the label says to give it as a separate injection and never mix the two; the same area is fine, but not right next to each other.1 For more on each site, see where to inject Ozempic.
A week with Ozempic, step by step
One way to keep the record; the amount, and any change to it, stays with your prescriber.

Ozempic can be taken at any time of day, with or without food, on the same day each week.1 Pick a day that fits your routine.
- 2On the day
Log the dose and the site together. Handle shows last week's site, so you can choose another.
- 3Through the week
Answer the daily check-in, and add a weight, meals or drinks whenever you like. Weights become weekly averages, so one heavy morning barely moves the line.
- 4If a week goes differently
Ozempic's instructions have rules for a missed dose and for changing your injection day, so ask your prescriber or pharmacist rather than guessing. Handle records what happened and never tells you to catch up.

Ozempic pill: how to track Ozempic tablets
In the US, Ozempic now names a tablet as well as the pen. Ozempic tablets are once-daily semaglutide tablets in 1.5, 4 and 9 mg strengths: a newer formulation first approved under the Rybelsus name, renamed in January 2026 and available from 4 May 2026.2,3
They are approved for adults with type 2 diabetes, including to lower the risk of major heart events in those at high risk.2 The original Rybelsus tablets (3, 7 and 14 mg) share the same US label, and outside the US the tablets are still sold as Rybelsus.2,3 The label calls the two "not substitutable on a mg-to-mg basis", so record the product name as well as the number.2
How the label says to take Ozempic tablets
- One tablet a day, in the morning, on an empty stomach.2
- With plain water only, up to 4 ounces.2
- Swallowed whole: not split, crushed, chewed or dissolved.2
- Then a wait of at least 30 minutes before food, other drinks or other oral medicines.2
- Stored at room temperature (20–25 °C) in the original bottle, away from moisture.2

Is Ozempic a pill or a shot?+
Both, in the US. Ozempic injection is a once-weekly pen. Since January 2026, the Ozempic name has also covered once-daily semaglutide tablets in 1.5, 4 and 9 mg strengths for type 2 diabetes, a formulation first approved as Rybelsus. Each comes with its own instructions for taking it.1,2

How long does Ozempic stay in your system?+
Semaglutide has a half-life of about a week, and the label says it stays in the circulation for about 5 weeks after the last dose.1 If you want a picture of that, Between doses (a Premium feature) estimates your level from your logged injections. Does Ozempic need to be refrigerated?+
Before first use, yes: pens go in the fridge at 2–8 °C and must never be frozen. After first use, a pen can be kept for up to 56 days at 15–30 °C or in the fridge.1 Ozempic tablets are stored at room temperature in their original bottle.2
Can I use an Ozempic tracker for weight loss?+
You can track your weight whatever the reason you take Ozempic, but weight management isn't one of its approved uses; Wegovy is the semaglutide injection approved for that.1,4 Handle charts your weight as weekly averages beside your dose record, with no streaks or scores.
