In short

  • A Zepbound tracker records each weekly shot (the amount, the day and the injection site) alongside how you feel and your weight trend, so appointments start from facts, not memory.
  • Zepbound is tirzepatide from Eli Lilly, a once-weekly injection approved in the US for long-term weight management in adults with obesity, or overweight with a weight-related condition, and for moderate to severe obstructive sleep apnea in adults with obesity.1
  • It comes in six strengths from 2.5 mg to 15 mg, as single-dose pens and vials and, since January 2026, as four-dose KwikPens and multi-dose vials.1
  • In the 72-week SURMOUNT-1 trial of adults without diabetes, average weight loss was 15.0% to 20.9% depending on the dose, against 3.1% on placebo, counting everyone who started (the treatment-regimen estimand).1,4

What to record in a Zepbound tracker

Keep it small enough to keep up. These details are the hardest to remember later and the most useful at appointments.

  • Every shot, as it happened. The amount, the day and the time. A late or missed week belongs in the record too; it is information, not a mark against you.
  • Where it went. The label lists the abdomen and thigh, or the back of the upper arm if someone else injects, and asks you to rotate sites with each dose.1
  • Which pen or vial. Note the date if you change form, because single-dose and four-dose forms have different storage rules.1
  • How you feel. Nausea, diarrhea, constipation, tiredness. In the trials, most people who stopped Zepbound because of side effects did so in the first few months, mainly because of stomach symptoms.1
  • Food, fluids and protein. Zepbound reduces appetite and food intake.2 Fluids matter when your stomach is unsettled: the label warns that fluid loss can injure the kidneys.1 See our GLP-1 food tracker guide and our guide to what to eat on Zepbound and Mounjaro.
  • Dates that matter. Starting, each dose change, any new medicine. On the contraceptive pill? The label recommends a non-oral method, or an added barrier method, for 4 weeks after starting and after each dose increase.1

Pens, vials and KwikPens: one record for all of them

Zepbound was approved in November 2023.2 Lilly began selling single-dose vials for self-pay through LillyDirect in August 2024, and in June 2025 announced vials in every strength.5,6 Two four-dose forms, a multi-dose vial and the KwikPen, joined the label in January 2026.1,7

The label says an opened KwikPen or multi-dose vial is thrown away after a total of 30 days at room temperature, 30 days after first use, or four weekly doses, even with medicine left, and that a KwikPen is never shared, even with a new needle.1 Handle's private note for the day is a handy place to write "new KwikPen today".

SURMOUNT-1 and SURMOUNT-2 figures count everyone randomized, whether or not they kept taking it (the treatment-regimen estimand).1,4 The label's SURMOUNT-4 figures also count everyone randomized at week 36.1 If your weight changes direction, that is a signal to discuss with your prescriber, not a failure. There is no right way to do this; here is one ordinary week.

    Check in, and weigh if you like

    Ordinary days show what normal looks like for you, and weekly averages mean one heavy morning doesn't dominate your trend.

    Zepbound's prescribing information and Medication Guide have their own rule for a missed dose, so ask your prescriber or pharmacist. Handle never tells you to catch up.

Questions people ask

Is there a Zepbound tracker app?

Yes. Handle works with Zepbound: it records each weekly shot and injection site, a short daily check-in, your weight, food and fluids, and a consultation report for your prescriber is among its Premium features. Does Lilly have its own Zepbound app?

Yes. Eli Lilly's free Lilly Health app says it supports people on Zepbound, Mounjaro or Foundayo. Its App Store listing, seen on September 23, 2026, describes a logbook, dosing reminders and charts to share with your healthcare provider, for US residents aged 18 and over.11 Handle is a separate app that records any GLP-1 medicine.

Is Handle's Zepbound tracker free?

Yes. Logging shots and injection sites, the daily check-in, weight, food, drinks and activity, and the Journey charts are all free, with no advertising. The Between doses estimate, the consultation report with share links, food photo scanning and iOS widgets are Premium features. You never need them to keep a complete record.

Can I use a spreadsheet as a Zepbound tracker?

Yes, if you keep it up. Give it columns for the date and time, the amount, the pen or vial, the injection site (left or right), symptoms and how strong they were, weight, and anything new such as a dose change. What a spreadsheet won't do on its own is remind you, chart weekly weight averages, or produce a summary your prescriber can read quickly.

Where should I inject Zepbound?

Zepbound's label lists the abdomen or thigh, or the back of the upper arm if another person gives the injection, and says to rotate injection sites with each dose.1 Handle records the site, left or right, with every shot, so your record shows where you went last time. If one area keeps reacting, mention it to your care team.

How long does Zepbound stay in your system?

Tirzepatide's half-life is about five to six days in people with overweight or obesity, and a steady level builds after about four weeks of weekly doses.1 Handle's Between doses view, a Premium feature, estimates your level from the doses you recorded. Is Zepbound the same as Mounjaro?

They contain the same medicine, tirzepatide, made by Eli Lilly. In the US, Zepbound is approved for weight management and sleep apnea, and Mounjaro for type 2 diabetes.1,10 In the UK and EU there is no Zepbound brand, and Mounjaro carries the weight-management license.9,12 Compare them in Mounjaro vs Zepbound.
