Wegovy pill vs injection
Wegovy pill vs injection: one medicine, two routines
Wegovy pill vs injection is a choice between two ways of taking the same medicine, semaglutide: a tablet every morning, or an injection once a week. Here is how they compare on trial results, the rules for taking each, side effects, storage and availability, straight from the US label they share, and how to keep a record of either.

In short
- The Wegovy pill and the Wegovy injection contain the same medicine, semaglutide, from Novo Nordisk: the pill is a tablet taken once a day, made in 1.5, 4, 9 and 25 mg, and the injection is given once a week.1
- In their main trials, adults with overweight or obesity and without diabetes lost 13.6% of their body weight on average over 64 weeks on the 25 mg pill (OASIS 4) and 14.9% over 68 weeks on the 2.4 mg injection (STEP 1), against 2.4% on placebo in each, counting everyone randomized. These were separate trials, not a head-to-head.1,4
- The label says to take the pill on an empty stomach in the morning with up to 4 ounces of plain water, then wait at least 30 minutes before eating, drinking or taking other oral medicines; the injection can be given at any time of day, with or without meals.1
- Side effects are the same kind, mainly nausea, vomiting, diarrhea and constipation, and the label says the pill's common side effects were similar in type and frequency to the injection's.1
- The pill was approved in the US on December 22, 2025, and in the UK on June 11, 2026. Which form suits you is your prescriber's call, and Handle can record either.2,5
Wegovy pill vs injection at a glance

US prescribing information for Wegovy injection and tablets, effective June 18, 2026. The two weight trials were separate, so their results are not a head-to-head comparison.1 How long semaglutide stays in the body after each form is covered in our Wegovy and Ozempic half-life guide.
Is the Wegovy pill the same as the injection?
It is the same medicine delivered a different way. Semaglutide is a peptide, and only a small fraction of a swallowed dose reaches the blood: the tablet is made with an absorption enhancer, SNAC, and about 1–2% of each dose is absorbed, mostly in the stomach.1 That is why the label's rules about food and water matter so much.
The label predicts that the 25 mg tablet gives average blood levels comparable to the 2.4 mg weekly injection in people without type 2 diabetes, though levels vary more from person to person.1 In people with type 2 diabetes, levels on the tablet were lower than on the 2.4 mg injection, and the label notes that some may have lower-than-needed levels; the tablets have not been studied for weight reduction in adults with type 2 diabetes.1
Some approvals apply to the injection only. The tablets are for adults, and they are not approved for MASH; the 7.2 mg Wegovy HD dose is also injection-only.1 Because both forms are semaglutide, the label says neither should be used together with the other, with any other semaglutide product, or with any other GLP-1 medicine.1
Wegovy pill vs injection for weight loss: the trials
Each form was tested against placebo in adults with obesity, or overweight plus a weight-related condition, who did not have diabetes. Neither trial compared the pill with the injection directly.
| Wegovy pill 25 mg (OASIS 4) | Wegovy injection 2.4 mg (STEP 1) | |
|---|---|---|
| Length | 64 weeks1 | 68 weeks1 |
| People | 307: 205 on the pill, 102 on placebo1 | 1,961 in total1 |
| Average weight change | −13.6% vs −2.4% on placebo1 | −14.9% vs −2.4% on placebo1,4 |
| Lost at least 5% | 76.3% vs 31.3%1 | 83.5% vs 31.1%1 |
| Lost at least 10% | 59.8% vs 14.4%1 | 66.1% vs 12.0%1 |
| Lost at least 15% | 47% vs 5.5%1 | 47.9% vs 4.8%1 |
| Lost at least 20% | 27.9% vs 3.1%1 | 30.2% vs 1.7%1 |
US label figures, counting everyone randomized whether or not they kept taking the medicine. Averages describe groups, not what any one person will lose. The trials differ in length and size, so the gaps between the columns are not a ranking.
You may see a bigger number for the pill elsewhere. Novo Nordisk reports about 16.6% for OASIS 4 if everyone had stayed on treatment, a different way of counting from the label's 13.6%, which includes people who stopped.2 Whichever figure you read, check which method it uses before comparing it with anything else.
How to take the Wegovy pill: the label's rules
Absorption happens mostly in the stomach, and food, drink and even extra water reduce it, so the US label is precise.1 These are the label's instructions, not extra advice from us.
- On an empty stomach, in the morning. One tablet, once a day, and never more than one tablet a day.1
- With plain water only. Up to 4 ounces (about 120 mL), and no other drinks.1
- Swallowed whole. Not split, crushed, chewed or dissolved.1
- Then wait at least 30 minutes. No food, no other drinks and no other oral medicines until the 30 minutes are up.1
In studies, more semaglutide was absorbed with less water and with a longer wait after the dose.1 The UK instructions go further, asking for a fast of at least 8 hours beforehand and just a sip of water; the US label does not state an 8-hour fast.5 Follow the instructions that come with your own pack.
How the Wegovy injection differs
The injection has no food or water rules. It is given once a week, on the same day each week, at any time of day, with or without meals, into the abdomen, thigh or upper arm, rotating sites with each dose.1 Most pens hold a single dose; the Wegovy FlexTouch pen holds four 2.4 mg doses.1 Our guide to Ozempic and Wegovy injection sites covers rotation.
Both forms start at a low strength and step up gradually, which the label links to fewer stomach side effects; your prescriber sets the steps. If a dose is missed, the pill and the injection each have their own rule in the label, and the two are different, so ask your prescriber or pharmacist rather than guessing. Handle records what actually happened and never tells you to catch up.
Wegovy pill vs injection side effects
The label says the pill's common side effects were similar in type and frequency to the injection's.1 The trial registry gives the pill's own figures, shown here beside the injection's label figures. They come from different trials and are counted differently, so read each column on its own.
| Side effect | Pill 25 mg (placebo) | Injection 2.4 mg (placebo) |
|---|---|---|
| Nausea | 46.6% (18.6%)6 | 44% (16%)1 |
| Vomiting | 30.9% (5.9%)6 | 24% (6%)1 |
| Constipation | 20.1% (9.8%)6 | 24% (11%)1 |
| Diarrhea | 17.6% (8.8%)6 | 30% (16%)1 |
| Indigestion (dyspepsia) | 18.1% (8.8%)6 | 9% (3%)1 |
| Belching (eructation) | 10.3% (2.0%)6 | 7% (under 1%)1 |
| Headache | 11.8% (8.8%)6 | 14% (10%)1 |
| Fatigue | 7.4% (1.0%)6 | 11% (5%)1 |
| Any stomach side effect | 74.0% (42.2%)3 | 73% (47%)1 |
| Stopped because of side effects | 6.9% (5.9%)1 | 6.8% (3.2%)1 |
Pill: OASIS 4, 204 people on the pill and 102 on placebo, from the results posted on ClinicalTrials.gov (weeks 0–71), the journal paper and the US label. Injection: US label, STEP 1–3 pooled, 2,116 people on Wegovy and 1,261 on placebo, up to 68 weeks. More in Wegovy side effects, which covers the pill too.
Because they share one label, the pill and the injection carry the same warnings: pancreatitis, gallbladder problems, kidney injury from dehydration, serious allergic reactions, heart-rate increase, and breathing in stomach contents under general anesthesia or deep sedation, so tell your care team before any planned procedure.1
The label gives a few figures for the tablets specifically: severe stomach side effects in 2% on the pill vs 0% on placebo, gallstones in 2.5% vs 1%, and dysesthesia, an altered or painful skin sensation, in 5% vs 0%.1 Two more tablet-only points: breastfeeding is not recommended while taking the tablets, because of SNAC, and in a study, taking oral semaglutide with levothyroxine, a thyroid medicine, raised thyroxine exposure by 33%, so tell your prescriber if you take it.1
In Handle
Write down how the weeks feel, pill or injection
Whichever form you take, the weeks after each step up are the ones worth recording. Log each dose as you take it, then answer a short daily check-in: nausea every day, plus the symptoms you choose. It stays the same short card whatever you answer.
- Vomiting, constipation, heartburn and more if you switch them on
- Up to six extra symptoms, or one in your own words
- A private note for the day that nothing reads

Wegovy pill or injection: what differs in practice
The trials above cannot tell you which form is better for you. These are the practical differences in the label, worth talking through with your prescriber.
- Needles or a morning routine. The injection means one injection a week. The pill means a tablet every morning, with the fasting and waiting rules above.1
- Other morning medicines. Nothing else by mouth for at least 30 minutes after the tablet, which matters if you take other medicines first thing, including levothyroxine.1
- Type 2 diabetes. The tablets have not been studied for weight reduction in adults with type 2 diabetes, and blood levels on them were lower in people with it.1
- Age and other conditions. Teenagers, MASH and the 7.2 mg dose are covered by the injection only.1
- Storage and travel. Tablets stay at room temperature in their bottle; injection pens belong in the fridge, with a limited time out of it.1
- Breastfeeding. Not recommended while taking the tablets.1
- Coverage. A plan may cover one form and not the other, so check before assuming either.
Ozempic pill vs shot
Ozempic has the same split, for type 2 diabetes rather than weight management. In the US, the newer oral semaglutide tablets were renamed Ozempic tablets in the label on January 30, 2026, and went on sale on May 4, 2026.9,11
Ozempic tablets
Once a day, for type 2 diabetes
- Strengths: 1.5, 4 and 9 mg. Rybelsus 3, 7 and 14 mg share the label, and the two are not interchangeable milligram for milligram9
- The label's morning rules match the Wegovy pill's: empty stomach, up to 4 ounces of plain water, then a wait of at least 30 minutes9
- For blood-sugar control and lowering heart risk in adults with type 2 diabetes9
- In the label's 26-week trial of the original tablet, HbA1c fell 1.2 points on 7 mg and 1.4 points on 14 mg, against 0.3 on placebo9
Ozempic injection
Once a week, for type 2 diabetes
Separate trials, so not a head-to-head. Outside the US, the tablet is still called Rybelsus.11 For the brand comparison, see Ozempic vs Wegovy.
Other GLP-1 pills and injections
The Wegovy pill is not the only GLP-1 tablet for weight management. Foundayo (orforglipron), from Eli Lilly, was approved in the US on April 1, 2026. It is a small molecule rather than a peptide, and Lilly describes it as a pill that can be taken at any time of day without food or water restrictions.12 See Foundayo vs Wegovy for that comparison.
If you are weighing the pill against a different weekly injection, see Zepbound vs Wegovy and Mounjaro vs Wegovy, or every GLP-1 medicine side by side.
In Handle
Log the pill or the injection in one place
Wegovy is in Handle's medicine list by name. The Wegovy pill is not listed yet: add it under "Something else" with its name, amount and daily schedule, and logging, the check-in, weight, food, Journey and the consultation report (part of Premium in the iPhone app) work the same way.
Record the dose you actually took, when and, for the injection, where. Between doses, also part of Premium, is an estimate of your medication level drawn from the doses you record; how you record the pill affects which features apply to it.
- The injection site with every injection
- Dose reminders in the iPhone app
- Forgot? Add entries up to 30 days back

Changing from one form to the other: what to record
The Wegovy label includes instructions for moving between the tablets and the injection, and applying them is your prescriber's job; this page does not cover doses or timing. What helps is a clear record.
- The last dose of the old form and the first of the new: date, name and strength, exactly as prescribed.
- Your new routine: when you take the pill and eat afterwards, or your injection day.
- How the next weeks feel: nausea, appetite and bowel changes, especially after each step.
- Your weight trend, as weekly averages across the change rather than a single morning.
Handle keeps both forms on one record, and the consultation report, part of Premium in the iPhone app, shows the change alongside your weight history and symptom summaries. More in the Wegovy tracker guide.
Cost, coverage and the UK
Coverage and cost vary by insurance plan, pharmacy and country, and they change often. Check with your plan or pharmacist before assuming either form is covered.
In the UK, the MHRA approved the Wegovy tablet on June 11, 2026, as the country's first GLP-1 tablet for weight loss, and it is not currently available on the NHS.5 NICE recommends the injection for NHS use only within specialist weight management services, for a maximum of two years, for adults who meet its criteria.7 Private prices vary by pharmacy.
Questions to bring to your prescriber
Bring these to your next appointment; there is more in preparing for a GLP-1 appointment.
- Would a daily pill or a weekly injection suit my routine and health better?
- I take other medicines in the morning, including thyroid medicine: how does the pill's 30-minute wait fit around them?
- I have type 2 diabetes: does that change which form makes sense?
- What should I do if I miss a dose of the form I'm on?
- Which side effects should I call you about?
- If I change form later, how will we plan and review it?
Questions people ask
Is the Wegovy pill as effective as the injection?+
No trial in the label compares them head to head. In separate trials in adults with overweight or obesity and without diabetes, people lost 13.6% of their body weight on average over 64 weeks on the 25 mg pill and 14.9% over 68 weeks on the 2.4 mg injection, against 2.4% on placebo in each, counting everyone randomized.1 The label predicts comparable average blood levels in people without type 2 diabetes, with more variation on the pill.1
What is the difference between the Wegovy pill and the injection?+
Both are semaglutide. The label says to take the pill every morning on an empty stomach with a little plain water, then wait at least 30 minutes before food, drink or other oral medicines; the injection is given once a week at any time of day.1 The injection is also approved for teenagers and MASH, and comes in a 7.2 mg dose; the pill is for adults only.1
Does the Wegovy pill have fewer side effects than the injection?+
Not according to the label, which says the pill's common side effects were similar in type and frequency to the injection's.1 In OASIS 4, 46.6% of people on the pill reported nausea, against 18.6% on placebo; in the injection's label trials, 44% did, against 16%.6,1 These come from different trials, so they are not a direct comparison.
Can I drink coffee after taking the Wegovy pill?+
Not straight away. The US label says to take the tablet with plain water only, up to 4 ounces, and then wait at least 30 minutes before eating, drinking any other beverage or taking other oral medicines.1 That includes coffee, tea and juice. If the wait is hard to fit around your mornings, talk it through with your prescriber or pharmacist.
Why does the Wegovy pill have to be taken on an empty stomach?+
Only about 1–2% of each tablet is absorbed, mostly in the stomach, with the help of an absorption enhancer called SNAC.1 In studies, more was absorbed with less water and with a longer wait before eating, which is why the label asks for an empty stomach, a little plain water and a wait of at least 30 minutes.1
Can you switch from the Wegovy injection to the pill?+
The Wegovy label includes instructions for moving between the injection and the tablets, so a switch is possible, but the plan belongs to your prescriber: they decide whether it suits you, which strength and when.1 Don't work it out on your own. Record the last dose of one form and the first of the other, so the change is easy to review.
Is the Wegovy pill available in the UK?+
It is approved there: the MHRA approved the Wegovy tablet on June 11, 2026, as the UK's first GLP-1 tablet for weight loss, but it is not currently available on the NHS.5 The UK instructions say to take it after fasting for at least 8 hours, with a sip of water, and then to wait at least 30 minutes before eating or drinking.5
Does the Wegovy pill need to be refrigerated?+
No. The tablets are stored at room temperature, 20–25 °C, in their original bottle to protect them from moisture.1 The injection is different: the pens are kept in the fridge, and a single-dose pen can be kept at 8–30 °C for up to 28 days if needed.1
Is there an Ozempic pill?+
Yes, in the US. Ozempic tablets (1.5, 4 and 9 mg) are oral semaglutide for adults with type 2 diabetes: the newer Rybelsus formulation, renamed in the label on January 30, 2026, and on sale from May 4, 2026.9,11 Their label gives the same morning rules as the Wegovy pill's. Outside the US, the tablet is called Rybelsus.11
